I can change my oil every hour if I want to, but Toyota says that I'm pushing the envelope if I don't change oil at least every 5K miles.
Insurance limits are MAXIMUMS

Medicare will pay for one "tissue removal from more than 6 digits" every 90 days.
However, unlike Toyota, they don't give any warnings about what will happen if a deeply-trained, learned person doesn't clip your toenails at least once in 90 days.
What they are really saying is "I don't care if you never do it, but I won't tolerate it more than once in 90 days."
So when he finishes his ritual, he digs down deep into the vast trove of knowledge accumulated over years of intense study and diagnoses that patient as needing more "tissue removal" in EXACTLY 90 days.
