>> If devolution of chunks of primary care towards pharmacies is cast as a positive change, you should expect primary care physicians gladly to take the opportunity to reduce hours and alter service offerings to improve their own personal and family lives, a change that international experience says never can be undone once it starts.

That would be a good thing, wouldn't it?
I think it would.
My VA MD is on a 5 day week and an 8 hour day. Unlike my former primary MD, he never runs in to see me out of breath after rushing from an emergency at the hospital.
He always sees me at the appointed time and never has to rush me out to meet some other unanticipated pressing need.
Why is that bad?
Anyone who does not go overboard- deserves to.
