Not unheard of. I know a thing that's been inserted into the building code, the screw which connects the grounding cable with the rod which is hammered into the soil. That screw costs $0,20 and has been used for decades. Now someone invented a safer screw, which comes with its little box with a thermite charge, so instead of screwing it you apply a match to the wick, and then it explodes inside the box, thus fusing the wire and the rod. Costs $20. The number of cases when the old screw caused any damage: about zero. But the exploding box is now in your building code.
