Here's something quite interesting:

http://www.chcf.org/~/media/MEDIA%20LIBRARY%20Files/PDF/PDF%20H/PDF%20HealthCareCosts15.pdf

or for an interesting interactive summary:

http://www.chcf.org/publications/2015/11/data-viz-hcc-national?_cldee=anIxQHBvLWR1bmsuY29t
(interesting trends in out-of-pocket, Medicaid, and Medicare proportion of physician/clinical in recent years)

Question: if "other payers" means "worksite health care, Indian Health Services, workers’ compensation, maternal and child health, and vocational rehabilitation" ... how socialized was the US hospital care market pre-ACA? How about physician/clinical services?
"... They ne'er cared for us
yet: suffer us to famish, and their store-houses
crammed with grain; make edicts for usury, to
support usurers; repeal daily any wholesome act
established against the rich, and provide more
piercing statutes daily, to chain up and restrain
the poor. If the wars eat us not up, they will; and
there's all the love they bear us.
"
-- Shakespeare: Coriolanus, Act 1, scene 1
