Even back in 2013 the President acknowledged that his promise went too far. Very few if any health plans fell into the grandfather rule. It was actually rather ridiculous. I personally know a few people who fell into the bucket of losing their plan and not qualifying for subsidies in order to afford the new plans so they now have nothing. They are not required to purchase a plan (couldn't afford it if they wanted to) because North Carolina and other states chose not to expand Medicaid. You could call that the fault of the state, but regardless, people I know lost healthcare when they, very naively, thought they could keep their plans. What really happened is the plans got cancelled and they were offered plans outside anything they could afford without a subsidy. So it did indeed happen and Obama and the White House acknowledged it.

A lot of these issue only popped up due to the back and forth negotiating and changing of the whole plan period. Had it allowed insurers to cross state lines and there to be a one payer option, it would have covered many more and been more affordable as well. What we ended up with is not close to what the initial goal was until big companies and insurance companies and lobbyists got involved and fought to bring it down so the insurance companies could continue to prosper.
