Well, when we lived in Akron, our PCP treated all kinds of patients including Medicare and Medicaid. I don't know what you mean by "treating the well off". What does "well off" mean? If it means that one can afford to retire, then I most certainly am not well off and she treated me ;)

Since we moved to Arkansas, we have seen new doctors and, interestingly enough, the new ones we have here in Arkansas fell the same way as the ones in Akron did.
