Actually about 5 minutes ago I think I figured the cause. The folder where the .exe resides does not seem to have (in Security tab of the properties) the user IIS_IUSR and USERS. I will ask the customer IT to add these users to the folder and give them Full privileges.
But I like your idea of coverage log which I will add, just in case.
