Nice paraphrases. You're saying that physicians should fight city hall because the public won't?

My experience is that physicians turn out to be human beings who can and should care about themselves and their families. When pushed too hard, especially if administrative/infighting overwhelms patient care: they walk. The same physician who fights an insurance company fiercely to secure a kidney for a young mother, will become demoralized and will start valuing greener grass elsewhere if they're also expected to fight insurance companies for rates of pay, for medication gouging and for all of the other entrenched special interests and their mercenaries.
"... They ne'er cared for us
yet: suffer us to famish, and their store-houses
crammed with grain; make edicts for usury, to
support usurers; repeal daily any wholesome act
established against the rich, and provide more
piercing statutes daily, to chain up and restrain
the poor. If the wars eat us not up, they will; and
there's all the love they bear us.
"
