'Tis true. For whatever reason, foreign physicians seem to get a pass on the whole class thing (in the UK as well.) I've seen pricing used to cut out whole swathes of population, followed by requirement for member sponsorship and black ball strategies that allow clubs to wring their hands that there's nothing they can do if somebody anonymously keeps dropping a black marble in the bag.
