VFP 9, Windows Server 2016, SQL Server 2008 or 2012?

Short answer, if you don't have any problems with 2008 R2, it should work as is with 2012 too.

To my experience VFP9 works with any SQL server versions from 2000 to 2016 without no (major) problems (used all editions except compact). There are few problems mostly related with new data types and varchar/varbinary(MAX) but you should already have them handled with 2008 R2, you shouldn't have any problems with 2012, 2014 or 2016. You may or may not want to use 2012 driver (it is fine to continue with 2008 driver, though I rarely experienced problems that it doesn't work as intended then - I give user the option to choose the driver when setting up the connection information).
As per the windows version, our customers have windows servers ranging from 2003 to 2012, but not sure after than that. Considering their numbers, I think at least a few should be using the latest and if had a problem they would surely let me know.
