Since I have no idea how you coded it, how you connect to/access MySQL what error handling you employ that is very had to guesss.
For paranoids or people under the watch of tax auditors it makes sometimes sense to move some "biz intelligence" server side, like flagging a certain field dependant on correct save via stored procs.
And having extensive logging available during testing can be a big help...
