>>..and there we have it -- clearly you did not read all of that (if any) yet -- so no point in talking further in this "anti-intellectual ad hominem conversation"

Playing the professor and setting homework for others may be fun, but why would any student (and I cheerfully call myself a student of healthcare funding) waste time on a pasted bibliography that *clearly* predates the December paper that you refuse to acknowledge? Maybe click a few of the links yourself and look at the dates?

Nor is it difficult to identify the source of your stats based on identical sequence, identical obsolete cohort numbers and some identical word-for-word- https://www.nytimes.com/2019/03/26/health/obamacare-trump-health.html - but that March 2019 lay article also clearly predates the December 2019 paper and is profoundly obsolete.
"... They ne'er cared for us
yet: suffer us to famish, and their store-houses
crammed with grain; make edicts for usury, to
support usurers; repeal daily any wholesome act
established against the rich, and provide more
piercing statutes daily, to chain up and restrain
the poor. If the wars eat us not up, they will; and
there's all the love they bear us.
"
-- Shakespeare: Coriolanus, Act 1, scene 1
