Summary:
Problem occurs when running INDEX ON from different records.
Problem is 20 years old and of no use since the invention of cursoradaptors. It was created to document views, and the table a left over in the master (meta) database.
Solved by deleting the table.
This is a win-win. Learned something and removed lava-code.
