Just my .02 here. I'll go with numbers 1, 4 and 5 on your list. Speed and proficiency come with familiarity both with the tools and the code.

I would also add the ability to recognize when you are getting 'wrapped around the axle'. By this I mean you've overthought the problem and made the code unreadable, confusing and clunky. I have been fortunate enough to recognize when this happens to me and be able to stop, step away for a minute and then come back at it 'fresh'. I've also inherited code from people who either didn't or couldn't see this happening yet the rest of their code is good. It's a beast of an issue.

Again, this is my opinion.
