After all, if we are going to punish those who do not wish to take this C19 medical treatment then we should also punish those people who willing do unhealthy or even dangerous things at the cost to social medical-care. Smokers are an obvious target. Then the obese. Then we need to stop the alcohol drinkers, meat eaters - in fact all non-vegetarians. The sugar consumers, fast-food consumers, cola drinkers, etc. Those who do not exercise. All extreme sports people, skateboarders, sky-divers, etc. I'm sure we can make up a good list to punish. This will create a huge cost saving to the state which I am sure they can put to much better use such as invading some country or other to bring them democracy.
