This seems like a decent primer on how to get started. Covers the major tools that popped into my head and gives you a solid foundation for a development stack.

I don't know about Form design IDE, that doesn't seem like a thing in JS. The development cycle seems like: (1) fiddle with the HTML/CSS (2) run your app in a browser to see how it looks (3) return to (1).
