I've seen my share of that, even recently, but can't pinpoint the exact situation when it applies. I sort of remember it was some kind of dependent procedure, called on a timer, execscript(), cursoradapter event code or some such not-so-straight way of call. It would appear when I'd click cancel in the debugger, but the form was still alive and I could close it regularly.
