No, you don't need to set up your own Exchange server. Microsoft is actively discouraging that these days. You would subscribe to an Exchange-powered cloud server from Microsoft or others. For business this would likely be Exchange Online, part of Microsoft 365 (M365).

I don't deal much with retail/consumer mail but I understand you can get a free account on outlook.com : https://outlook.live.com/owa/ . I believe this also uses an Exchange back end.
