Hopefully this is hypothetical...but some how I doubt that. :-)

There are several ways of dealing with this, and I think all of them involve designing the form to fit in 640x480 (actually you'll probably have to settle for 600x450, or there abouts, due to borders, start menu and what not).

A couple of ways would be to use:
Pageframes; Multiple Forms; Overlaying fields and toggling the visible attribute. That last option can be really annoying and would be my last choice, but that may be what the user wants.
