Which HLPTOHTML did you use? I've played with a few last year and I didn't obtain as much result.
On my side, since I will start from scratch for my first help in my next application, I will just put them in a table and use my table help generator. In that way, I control everything related to the result and all the hyperlinks, see also, etc. are being proceesed as I want them to be.
