You picked a pretty wide-open question. I can think of a number of resources. I thought that the BOL covered each subject pretty well. MS has a class, #833 - Implementing a database design on MS SQL Server 7.0, which goes into both of those subjects and more (I'm teach that class this week).

General questions could be answered by any SQL Server book.
