You don't need a VFP form on the client side to do this, as a matter of fact, I wouldn't recomend it. A simple HTML form will do the trick. The tricky part is what happens on the server side. I could write a few paragraphs (or pages, or chapters) listing all of your options, but I'll save my time and yours by pointing you to The West Wind Technologies website.
