Hi Michelle. I just tried this on my form class that is for reports and the icons appeared fine (on all 35 reports). It most likely is that the icons on the forms where changed and then changed back at one point so that the icon property no longer says "default". Try it on one form and right-click on the icon propeter and click "default". This should do it.

Don't worry, this caught me once too.
