I am just starting to work on something similar. The solution I a attempting is to use an ActixeX called Spread 3.0. You can download it from www.componentsource.com.

This control is a lot like Excel and allows me to set attribites like color and font on a cell by cell basis. I can also lock out many cells from getting the focus and draw boxes, etc around rectuangula ranges of cells. You can address 'regions' by using Excel-like row/column coordinates, and trap for cursor/mouse entry.

I am setting it up to not show grid lines to you can see it is a spreadsheet.

It also supports OLE Drag & Drop which is how I am allowing the user to drop questions on the questionnaire form.

Download it (7 megs.) and see if it can do what you want. I am happy so far.

The columns would contain either vertically sizeable, editable text fields or diagrams with cut and paste and edit capabilites. Also, any column+row (rectangular) region on the form could have "hot-spots" on the form for allowing the user to show popup windows for selecting valid items for that region. And of course it would print as seen on the screen.
