Form classes do not have a DE associated with them. That's the job of a form object (subtle difference, I guess).

You can create a form based on your form class and fill in the DE there.

P.S. You don't get good exposure to your questions if you post them in the middle of another (even related) thread. :-) A lot of folks only scan top level and dig deeper if they are interested or have some free time.
