I rely on Stonefield's SDT to manage the process; it's got a wonderful set of tools for propagating changes to a .DBC via metadata, including altering table structures, adding/removing/redefining indexes and view, as well as good tools to perform common database maintenance tasks in a very reliable fashion. it can be incorporated into an application and distributed on a royalty-free basis. Stonefield is a member of the UT partner Program, so there are discounts for UT members, and there's a support message area here for their products.
