In Windows 3.1 help for Windows itself, there was a topic about keyboard shortcuts. It was documented there. I think that's also where I learned that Ctrl-F4 meant close a window within the app, as opposed to Alt-F4 closing the app. (Or maybe I just noticed that while looking at a window's control menu in Word or Excel.)
