The user don't have to uninstall the old application before installing the new one, unless the old application used the registry to save information and the new one don't use the same key or you used an ActiveX in the old version that isn't used in the new version. In that case, yes it's better to do an uninstall first so that the machine get cleaned.

If you just made a small change in the application (didn't modify any access to the registry, didn't add new ActiveX controls), then you can only distribute the new EXE to copy over the old one.
