I think you need 48X48 format as well. This to have the ICO show in all possible views in the explorer.

But what I am thinking about as well:
The Icon attached to the project is the one that shows up with the application running.
The one attached with the setup shows up within the Windows Explorer.
If I'm right!

Depending on where you want your icon, attach it in the appropriate place.
