We have dealt with UUNET as our ISP. In the beginning we started with Frame Relay. This required buying a Cisco 2501 router and a Kentronix CSU/DSU, which combined were about $2500. I think Cisco has released models since then that do both. The total monthly cost of the service (UUNET + Telco) was around $500.

When our bandwidth needs increased, we switched to "burstable T-1" which allows you to have full T-1, but you're billed according to your usage, thus allowing you to scale up gradually. We were able to use the same Cisco router, and only had to purchase a different CSU/DSU. We're now paying around $1800/month, which is about half of what a full T-1 would be.
