We are using xCASE4 -
The biggest advantage of this tool is that it works with VFP while other tools
(ERWin 4.0 due in mid 99 is supposed to have support for VFP as well but doesn't support it now)

In ver. 4 xCase also supports transalting from database model to database model
For some DBMSs it does a good job (e.g. UDB) for others we've seen better (e.g. Oracle) other tools such as the above mentioned ERWin support more backend types

MMI wise xCASE doesn't look vey slick but is functional although it can be annoying at times

Btw, I haven't looked at Visio for awhile but Visio 5.0 pro/enterprize version also has database diagraming capbilities
