On your problem with creating a second form:

The problem that you are having is caused by the fact that the variable for your second form is losing scope and so the form 'disappears' when the variable does.

You can either set the variable to 'PUBLIC' before doing the createobject or use a property on your first form (or application class) for the createobject statement.
