I have posted another version of this question...but now I have kind of better defined the problem.

I am using the setup wizard for the first time in VFP 6.0. When the setup
finishes, the icon for my application that shows when I go from the start menu to the programs menu to my application...is a fox head. If I go to the
properties of my application's shortcut the icon filename is ls.exe rather
than ls.ico. If I change the icon filename to ls.ico in the properties of
the shortcut everything works great. Can someone point me to what I am doing
wrong. The only place I am setting up the icon is in the project info
screen. I am choosing ls.ico for the icon file.

This all worked fine in VFP 5.0.
