In NT to set a custom form you must first have full control permission. You then go to the Printers Window and highlight your printer you want to set custom paper size for. Then click on file and server properties. On the forms tab there is a check box for create a new form put in the name of your form and it's dimensions and click OK. For your user to be able to change the paper size they too will have to have full control of at least that printer. You can do this by right clicking on the printer and putting the users account in the permission with full control. Note if this is an nt workstation in a NT domain be sure to put the users domain account in the printer's permisions. I hope this helps
