The only times I've seen this are (1) attempting to create a directory with a long file name (not 8.3 compliant, or with spaces in it, on an older Novell file server that didn't have the OS/2 namespace loaded), (2) attempting to create a directory on read-only media, or a mapped drive or UNC where the user didn't have write privileges, or (3) where there was a file that matched some part of the path that needed to be allocated, e.g. you can't create the directory C:\Apple\Orange\Banana if there's a file named C:\Apple, C:\Apple\Orange or C:\Apple\Orange\Banana - a directory is just a file, and two files can't have the same name.
