I've seen similar behavior tied to specific video drivers (the ATI Rage cards left window droppings behind in some early driver versions, for example); does this happen regardless of the video card and video mode of operation? To try to rule out drivers as an issue, try using the standard 640x480 VGA driver on the system (I know, it doesn't look good, but it's just for testing) and run the app; under NT, simply boot to the standard VGA option in the NT boot manager, and under Win95, you can try running in Safe Mode. If the problem goes away, and more importantly, if it doesn't happen on other systems using a different card and/or driver, then I'd tried to find a different video driver for your video card, or a different card and driver.
