Hi Potter, this sounds like a good solution to this but I don't see where
you are seeing an option for "Custom" paper size. In the Windows NT
Print Setup panel, I see something like:

Printer section:
* Name, Status, Type, Where, Comment and [Properties]

Paper section:
* Size "Letter", Source "Automatically Select".

Orientation:
* Portrait, Landscape

Under the Paper section, the Size combobox has:

A4 - A5
B5
Envelope XXX where XXX is a number of different non-obvious choices
Executive
Legal
Letter
Letter Small
Note

However, I see no "Custom" option??
