I don't think it can or should be done, except possibly by API, since these are under the control of Windows. You should ask, for example, what if there's a lockup and you need to gracefully exit, but CTRL+ALT+DELETE is not available. You will have to turn off the machine to get out, which may cause data loss/corruption, etc...
