Heh, heh, heh. I was cruising the UT and the MS newsgroup...and came across your message and a similar one that Arnon Gal-Oz answered. Here's his post:
Your problem is that your form is hidden at the time you try to add the
toolbar to it

A toolbar that is set to show "in top level form" will appear in the active
form at the time it is initiated
since in your case that is the _screen this is where it appears
you can initiate the toolbar when the form is active, you should also be
able to ACTIVATE the toolbar in your form instead
