I need o restructure my rates database, because theway that it is now i will not be able to get what i wnat...right now its like this

011575 Colombia .23 .25 .34
011875 El Salvador .66 .43 .50

they are about 700 records...i need some how with a formula or function or variable determine the rate
I, the enduser set the rate for the customer, persay john has rate2
so, calls made to 011575657556 was made to colombia at the Rate2...
how do i do this..
i need to let the program know that is going to use the field 2 for rate2..or three for rate3
the database i got it from my boss and he does not want to modify it..
what i was trying to do is modify the database but its very har to doit...
because there is going to be lika 2000 records that i would need to put in again..
and if i did it it woud look like this
