This remids me of every now and then, when I get slammed, and my phone companies sends me this form to fill out so they must get written permision.

Me: But I filled out that form three months ago.

Them: We have no record of that sir.

Me: But I filled it out.

Them: Sorry, we will send you a form to fill out.

Me: Ok...

Seems to me, that THEY should be signing a form and promising me that they won't change it without my written permission.
