Could it be this mainmenu.scx form is not unloading properly? After you Clear All, m._rval should get released, and so the form it refers to should release itself. Now if that form is unsuccessfully unloading, i.e. having a dangling object referrence, or uncommitted changes... well, that's where I'd start looking at.
