To start would limit to drop down list boxes, option groups, text boxes and check boxes I think. On process form button on generated form would take text from all objects and concatenate it into a free text entry.

Sample

Drop down box says cough productive of green/yellow/bloody sputum etc.

Then in comment section store code to generate free text.

Idea is to create a pen driven type of form to generate free text but
have it all user definable.
