In addition to Craig and Paul's suggestions, you might find "The Official Microsoft HTML Help Authoring Kit" from Microsoft Press helpful. The examples are based on the earlier version (1.1?) of the Microsoft HTML Help Workshop so if you're using version 1.2 the screen shots and descriptions in the book are a little out of date.
