Whil Hentzen actually did a session (I think in '97) on consulting issues. He discussed many of the issues you bring up. I personnally thought it was a good session but I do not know what the overall response was to it.
It's definitely a great topic. One of the things I remember about Whil's session was how eager the attendees were to share their experiences with each other after the session.
I wonder if there would be a way of making it a panel session. You could select several issues (marketing, expectations management, proposals, etc.) and have three or four speakers discuss how they each address the issue.
