If this is happening on a WIn95 box or WinNT box that is pre NT Sp3, check to see if DCOM is properly installed. There is a FAQ entry here on UT that I wrote that tells you how to check to see if DCOM is installed, and there are detailed instructions on what to do for each type of operating system in the event that it is not there.
