Geez, I've been going at it the wrong way if you all are getting paid.

Seriously, I do my invoices differently depending on the contract I have with my client. If it's a 'line item' type of job where I get paid for each task then I'd write it out like suggest below. Each task either has a flat fee (per the contract) for a time period spent on that task with a dollar amount per hour or whatever. For other type of work I might simply put down how many hours worked and then the dollar amount. One line and it's all done. For my retainer-type jobs I just put down the month and amount and maybe some mention of the things I did for that month. I guess it really just comes down to what your client expects.
