I have this problem a lot, i havn't used paradox so i cant say it was or wasnt caused by paradox in your situation.
the problem is theat the links between the table file and the memo file have become jumbled.
couple of things to try,

see if you can pack the memo, you may get 'memo file is missing' message, if this happens try note below:-
create table from existing structure
append records from existing table to new table
view new table and see if this fixes the problem (pesimist probably wont in this situation)

if this does not work the only way i have found in the past is to restore from backup or manually reneter the data (not feasable in most cases)

the problem i have found is mainly due to file bloat, lack of available hard disk space when adding memo's or when reindexing, packing files etc.

In a current system i have a log which records individual notes entries in a sepearte table, i can then rebuild the notes for a client , worker etc from this table, bit more overhead but when users have entered 50 pages of notes for a client it is worth the overheads.
