Since you have SCREEN=OFF in your CONFIG.FPW, you need to either have a TOP-LEVEL form, or do an _SCREEN.Visible=.T. somewhere in your code. What's the reason for having SCREEN=OFF, is it because you want to run outside the VFP environment, or because you want to tweak some screen settings without watching it happen?
