I'd go with the web-based approach since it scales much better (and you have to do the rewrite anyways). I'd suggest the Citrix approach only if you had an existing app that needed to go multiple site. Take a look at West Wind if you decide to go the web-based route.

You can either connect your server to the internet or, what may work better for this client depending on the situation, use your NT server plus a bunch of modems to support the remote users. Setting up RAS under NT isn't all that bad. Depending on how often people are connected you may not even need a phone line/modem for each customer. Install a PC-Anywhere type program on the server for remote maintenance and you'll be good to go.
